Forms and Information for Incoming Freshmen

If you have an 8th grade student that is interested in attending Immaculate Conception High School, we ask that you download the forms below, have them completed and return the forms and all requested information to ICHS.  Once received, the Admissions Committee will review the application and you will receive a call from the high school.

Please note:  The Admissions Committee cannot review an application until all the forms and requested information has been received.
