TRANSFER STUDENT

Thank you for your interest in Immaculate Conception High School. Review the information outlined in the checklist and click on the links to the forms below. After completion, all forms must be sent to Jean Field in the ICHS Guidance Office at fax 630.530.2290 or jfield@ichsknights.org. Once received, the transfer application will be reviewed for acceptance.

In addition, the prospective student and family will be required to meet with the principal, prior to acceptance. 

ICHS does not accept senior transfer students, except for out-of-state transfers upon review by the principal.

• Enrollment Application

• Release of Information Form
  • Complete top portion of form
  • Submit form to current high school

• Transcripts (from current high school)

• Recent Standardized Test Scores

• Discipline Record Form
  • Complete top portion of form and sign
  • Submit form to current high school
  • Completed form must be returned to ICHS by current high school

NOTE: It is the responsibility of the parents to see the items listed above are sent to Immaculate Conception High School.

Students interested in a transfer to Immaculate Conception High School may want to consider spending a day at our school.  Please call 630-530-3484 to make shadow day arrangements.  The shadow day form must be completed and brought on the day of your visit.  [Shadow Day Form for Transfer Students]
